Where do I report my COVID vaccination for my employer-based incentive?

To report getting your COVID vaccine for your employer-based incentive, log into your online HealthEquity account and look for the COVID vaccination Incentive Program on your HQY dashboard. Click on the Incentive tab and follow the steps to complete your vaccination report.

If you do not see the Vaccination incentive on the main dashboard, you should reach out to your benefits team for instruction.
