To use your FSA funds and get reimbursed, you want to make sure your receipts and documentations are easily verified and processed.

Claim payment options
Log into your online account, select from options: Reimburse Me, Pay Provider, View Claims and Activity, Pick and Process to find the payment you need to make or claim to have paid. Follow prompts on screen to enter information according to which reimbursement or payment you need to make.
Submit a receipt for reimbursement
Log into your online account
Select Reimburse Me
Follow the steps to upload your receipt and complete all required information
Submit claim
Documentation must include all of the following:
Name of patient
Provider
Service or item purchased
Service date(s)
Dollar amount
You may also need to submit a letter of medical necessity for certain purchases.
What information does my receipt need to show?
Receipts and documentation must contain specific information about the services and products you are requesting for reimbursement.
We may ask you for an itemized receipt to verify that your transaction was for an eligible expense per IRS guidelines. An itemized receipt is a receipt, or documentation, that contains the following five pieces of information: Patient's Name, Provider's Name, Date of Service, Type of Service, Cost.
Why was my receipt denied?
In your account, under View Claims & Payments, if you click on a denied claim, there will be a specific reason for why the claim has been denied. Many claims are denied because of inadequate information on receipts.
Your receipt or documentation must contain five key pieces of information: Patient's Name, Provider's Name, Date of Service, Type of Service, and Cost.
